To join the SOS clinic, please first complete the simple self-referral form below.

This is to ensure your need are met, as Physiotherapy is a Healthcare Profession and has certain safety criteria. If Ciara needs to clarify any answers regarding your medical history or symptoms, please expect a phonecall prior to the clinic.

​You can then send either Julie or Ciara a message to get booked into a date.
